Performance Integrated Health offers direct billing to most Extended Benefits Plans!
We directly bill on behalf of patients covered by the following extended health care plans:

*Note: Some plans do not allow for direct billing. We are unable to view your unique plan coverage or pre-determinations. All claims will submitted at the time of the appointment at which time we can advise you of your plan's Explanation of Benefits for the service date.

Important information for Patients:
In order to be set up for Direct Billing, patients must provide their Member ID and Policy under any of the above plans. (Unfortunately, we cannot search patients without their member and policy information. The direct billing portal doesn't allow for it or provide any extensive plan information) It's imperative that patients have the information with them when they come to the clinic for initial set-up.
All patients need to sign a consent form authorizing the clinic to directly bill, as well as authorizing the clinic to receive payment, rather than the patient.
At this time, we offer direct billing to the patients primary insurer only. Any balance not covered via your primary extended health care plan will be billed privately at the time of your visit, and we will provide you with a detailed receipt to submit to your secondary insurer if applicable.
Example: Bob has Canada Life extended health coverage, and his partner has Pacific Blue Cross. They are on each other's plans. We are only able to submit to Canada Life for Bob, and he is able to submit the remaining uninsured amount to his spouse's plan privately if he chooses to do so.
We are unable to directly bill any user fees or fees for services rendered that are related to an open ICBC or Worksafe BC claim. This is an insurer policy.
